Appendix 2 - Bolnhurst and Keysoe Playing Field Risk Assessment Report 2008
Performed: - Monday, August 11th. Inspectors: - Carole Turner, Margaret Whitmore, John Worth.
- Field access: in good condition apart from the LHS gate supporting post which has snapped off and is leaning against village hall wall along with smaller section of gate. This was considered to be a hazard, probability of injury rated at 5 points, severity of injury rated at 4 points, therefore severity rating 20 points, high risk. Suggested Action to be taken, remove and repair as soon as possible.
- Notices: in good condition and secure. However it was wondered whether a notice should be put on the side gate into the shed area asking people to ensure that this gate is kept closed?
- New play equipment: has only just passed a ROSPRA safety inspection so no problems.
- Rubber impact absorbing matting: Installed so that edges around the matted area are tucked into the ground. However there is one point along the walkway down from the gate where the matting narrows and is not secured correctly creating a trip hazard. Probability of injury rating 3, severity of injury rated 2, overall rating 6 - low risk. Suggested Action to be taken, report to installers and get some additional matting fitted.
- Football Posts: It was noted that when the posts are lifted out of their ground sockets, one socket, out of three, is raised above ground level making a trip hazard. Probability and severity ratings 3 &2 respectively, overall 6 – low risk. Suggested Action to be taken, report to installers for resetting.
- General grassed area: closely mown and in good condition apart from a small pile of rubble which was immediately removed by the inspectors.
- Fencing: In good condition.
- Hall overflow outlet: is projecting out of the wall of the hall near the side gate. It is set at a height which could damage smaller children’s faces. Probability 3, severity 3, overall 9 – medium risk. Suggested action to be taken, glue an elbow over end of protruding pipe.
- Old branches to one side of main gates: These branches are situated on Village Hall land inside the play area, and present a hazard to any child falling into them. Probability of injury 5, severity 3, overall risk 15 – high risk. Suggested action to be taken, Ask village hall committee to arrange for their removal on safety grounds.
